Application Submittal

Please review the following information and gather the required documentation and materials.

Application packets received after the deadlines listed below will not be considered. Proposals must be submitted through the UWCF Online Submission System. No faxed, emailed, or mailed proposals will be accepted. Allow ample time for uploading and confirmation receipt. If for any reason a proposal is submitted twice, the last submission received will be the one accepted for review.

UWCF advises uploading proposal documents several hours prior to the deadline in case you encounter an issue with your internet connectivity. UWCF is not responsible for ensuring that applications are received by the deadline. If you encounter issues with the online submission system, please email wellness@uwcf.org.

For Returning Community Wellness Partners and New RFP Applicants

Before you begin gathering the required documents necessary to complete your application, be sure to review all the information below.

New & Returning RFP Applicants – Application Requirements

Throughout the application process, all organizations who are applying to receive Community Wellness funding will be required to provide the following:

501(c)(3) Determination Letter

Attach proof of your organization’s 501 (c)(3) tax-exempt status.

Proposed Program Budget

Download and Complete the Proposed Program Budget form. Be sure to save the file to your desktop prior to filling it in.

Review List of Unallowable Costs.

Proposed Personnel Detail Budget

Download and Complete the Personnel Detail Budget form. Be sure to save the file to your desktop prior to filling it in.

Proposed Fee for Service Schedule

Download and complete the Proposed Fee for Service Schedule form.

Signed Letter of Significant Partnership

If applicable, you’ll need to upload a signed letter of significant partnership.

Collaborative Partner Documentation

(If applicable)

Financial Documentation

You will be required to upload all of the following financial documents:

Financial Statement (Signed by CFO or Board Treasurer)
W-9 (download here)
Audit Reports
Form 990
Certificate of Commercial Liability Insurance
Most Recent Income Statement & Statement of Cash Flows

Additional Documentation

If you are proposing:

  • A new service(s): Attach a start-up timeline for each service
  • An expansion of services: Provide successful data of current program to expand
  • Continuation of a program that will end due to complete loss of funding: Provide the success of program-must be empirically demonstrated. Please explain the loss of funding for the continuation of program.
  • A significant partnership with another agency: Attach a signed letter of commitment from that agency’s Director or other authorized representative

Application Response & Rating Criteria

Text Box responses are limited to 300 words per question.  The RFP Selection Review Committee may request additional information in order to clarify your answers to questions, either in writing or in-person. Scores will be applied on each category, with a total scoring not to exceed 100.
