Organization Requirements

Organizations submitting an application must meet the following criteria:

  1. Organization must be able to provide a 501(c)3 Determination Letter
  2. Organization must answer each application question completely.
    • Do not include any materials not requested with your application, or any private personal identifiable information that would constitute special handling or breach of confidentiality.
  3. For Polk County resident(s) following a one-time service/program, the organization must ensure the reoccurring proposed service/program will be for:
    • Gross family/household income at or below 200% of the Federal Poverty Level Guidelines.
    • Documented full-time resident of Polk County (See Proof of Residency Guidelines)
    • Have no other health insurance, including Medicaid, Medicare, Polk Healthcare Plan, Veteran’s Health Benefits, or any private insurance.

      Exception: Residents of Polk County who qualify by residency and household income with health coverage, would be allowed to participate in a UWCF Community Wellness program IF:

        • The benefit being offered by a UWCF Community Wellness program is not a covered benefit under their plan OR
        • The individual has exceeded the plan’s benefit limitations for type of benefit/service. Adequate proof of non-coverage or coverage limits met must be provided.
  4. Organization understands there will be mandatory data collection of program expenditures and eligibility determination in a format provided by UWCF and monthly reporting requirements by the 15th of each month for the previous months’ activity. Additionally, a monthly digital submission of supporting documentation for all monthly expenses reported.
  5. Organization agrees to submit application in required format. Applications that do not follow the required format may lose points.
    • Late proposals will not be accepted or reviewed for funding consideration.
  6. Organization has reviewed the Florida Statutes 212.055(7) and Polk County Ordinance 2015-76, as amended.

Organization Requirements

  • For Polk County resident(s) following a one-time service/program, the organization must ensure the reoccurring proposed service/program will be for:
    • Gross family/household income at or below 200% of the Federal Poverty Level Guidelines.
    • Documented full-time resident of Polk County (See Proof of Residency Guidelines)
    • Have no other health insurance or the availability of health coverage, including Medicaid, Medicare, Polk Healthcare Plan, Veteran’s Health Benefits, or any private insurance.
  • Organization understands there will be mandatory data collection of eligibility determination and monthly reporting requirements.
  • Organization understands record retention (digital acceptable) is required for a ten (10) year period.
  • Organization agrees to submit application in required format. Applications that do not follow the required format may lose points.
    • Late proposals will not be accepted or reviewed for funding consideration.
  • Organization has reviewed the Florida Statutes 212.005(7) and Polk County Ordinance 2015-76, as amended.

Program Requirements

UWCF is partnering with Polk County Health and Human Service Community Health Care program to bring new opportunities to qualifying residents of Polk County, for behavioral health programs/services and preventative/early intervention programs/services to individuals and family members as well as healthcare coverage.

Polk County Health and Human Services Community Health Care (CHC) program administration has $9.6 million in allocated CHC funding to the UWCF Community Wellness Initiative program (Florida Statues 212.055 (7); Polk County Ordinance 2015-76, as amended).

UWCF desires to contract with agencies/organizations to provide programs/services, for behavioral health and prevention/early intervention, to reduce health care disparities to individuals and families in Polk County.

The focus of the initiative is to provide programs and services with a health component which serve residents of Polk County without healthcare insurance/coverage with incomes at or below 200% of the Federal Poverty Level. With this initiative, we are proposing to align community-wide efforts to achieve greater impact through collaboration through the following objectives:

Prevention/Early Intervention  |  Behavioral Health

$50,000 minimum requested amount for proposed program/services.

Prevention / Early Intervention: Programs and services that provide active, lifelong process of becoming aware of and making choices toward a healthier and fulfilling life. Prevention / Early Intervention programs and services do not have to include a Behavioral Health component but must have a “health” component.  Examples of programs and services but not limited to: physical activity, wellness, fitness, nutrition and/or health nutrition education, etc. 

Behavioral Health: Programs meant to improve the psychological, emotional wellbeing of Polk County residents.

Rent/Mortgage (Housing) Assistance

Utility Assistance

Childcare

Capital, Rent/Lease/Purchase/Building of facilities/Vehicles
