The form below is intended for Returning Partners who applied and received funding during the Pilot Year of the Community Wellness Program. The application below must be completed and submitted in full by June 30, 2023.

If your agency is a New RFP Applicant, please do not complete the below application. New RFP Applicants’ applications will be accepted from July 1 to July 31, 2023. Please review the Required Documentation to gather necessary documents and prepare your application.
